The end of orthodontic postgraduate training often brings confidence in avoiding significant diagnostic and treatment-related errors. Yet, as years of practice unfold, the reality of managing complex cases and unforeseen challenges becomes evident. High bracket loss rates, retainer failures, frustrating treatment outcomes, or relapses are just some of the difficulties that orthodontists encounter. Compounding these issues is the shift in orthodontic practice towards managing severe malocclusions, displaced teeth, skeletal anomalies, adult cases requiring combined orthodontic-maxillofacial surgery, and pre-prosthetic treatments.
In this lecture, I will share some of the most significant and challenging errors I have faced over the past 15+ years, along with evidence-based strategies and practical solutions to mitigate them. Drawing from clinical examples and current literature, we will explore pivotal diagnostic dilemmas, such as extraction versus non-extraction and surgical versus conservative treatment for skeletal anomalies.
Additionally, therapeutic challenges like bracket positioning, archwire selection, and the integration of devices such as Temporary Anchorage Devices (TADs) and Class II appliances will be critically analyzed. Finally, we will address the critical question of long-term treatment stability and the efficacy of various retention devices.
This session offers an honest look at the complexities of modern orthodontic practice and aims to equip participants with insights to anticipate, navigate, and resolve common pitfalls in achieving optimal patient outcomes.

Orthodontic treatment operates within the intricate environment of periodontal tissues, relying on the periodontium to transmit forces from teeth to bone. This dynamic process facilitates the remodeling of hard and soft tissues essential for effective tooth movement.
A comprehensive evaluation of skeletal, dental, aesthetic, periodontal, and "bone housing" factors is critical in the treatment planning phase. Combined with appropriate biomechanics, this approach ensures not only the correction of malocclusion and enhancement of smile aesthetics but also effective risk management. However, underestimating the impact of dental movements or mechanical forces on the periodontium can lead to serious complications, including attachment loss, gingival recession, and root resorption—challenges that are not always easy to resolve.
This presentation will explore various critical scenarios, highlighting common errors in treatment planning and mechanics. Practical solutions and strategies will be discussed, enabling clinicians to achieve successful outcomes while preserving periodontal health.
